Project Notes/Comments:

This session discusses the Diabetes Youth and Parent Group (DYPG), established in 2019 and designed to provide education, resources and support for high-risk youth with type 1 and their families. The pandemic, contributed to feelings of isolation children with diabetes already struggle with and exacerbated need to continue bestowing the services offered by the DYPG. This presentation outlines the changes developed by the multidisciplinary team to transition to a virtual format, build sessions that engaged youth and families with peers and outlines methods that increased participation and attendance. In addition, presenters will review data for attendance and motivation factors for members to return. It will also provide insight into evidence for ongoing development and the transition back to in-person sessions

Does the work incorporate equity?

This poster highlights a nurse driven program that focuses on a high-risk group of youth and families living with type 1 diabetes. There are no exclusion criteria. Even if a child had type 2 diabetes, they are welcome to attend. DCFS, CPS other pediatric hospitals reach out to us to have their kids and families join our group. All are welcome to participate.

Does this work address any of the UChicago Medicine quality priorities as listed on the Clinical Excellence Scorecard?

Hospital Readmissions, Ambulatory Access to Care, Patient Experience, Diabetes Management
