Does the work incorporate equity?
The Comprehensive Care Program is designed to provide additional support to patients who are medically and socially vulnerable. Patients in the program receive individualized care from an interdisciplinary team that offers physical and behavioral healthcare as well as social support. The CCP program has equity integrated into its design and is an attempt to provide high-quality healthcare to all patients in the program. The program's unique staffing model allows for an individualized approach to healthcare offering equitable care as determined by the patient and their team on a case-by-case basis.
